Abstract
Introduction: Stroke is one of the leading causes of death in Malaysia. It must be rapidly diagnosed and effectively treated to improve the clinical outcome. There was scarce data reported on the usefulness of the platelet parameters particularly platelet count (PC) and mean platelet volume (MPV) in assisting the acute ischemic stroke (AIS) treatment approach. Objective: This study aims to evaluate platelet parameters and MRI findings in facilitating a treatment approach in patients with AIS. This study also aimed to evaluate the usefulness of the parameters with the currently used NIHSS score in guiding the treatment. Methodology: A retrospective study was conducted using a simple random sampling method to choose patients with acute ischaemic stroke in Hospital Pengajar Universiti Putra Malaysia (HPUPM) from January 2020 till March 2021. This study used secondary data (patients’ demographic data, platelet count and MRI findings) which were extracted from the Hospital Information System(eHIS). All collected data were transferred and analysed using the Statistical Package for Social Science Program (SPSS) version 26. Result: It is found that most of the acute stroke patients in this study were male, Malay with a ranging age of 55 to 64 years old. Most of them had a moderate stroke, underlying medical illness and were on other treatment than thrombolysis. There was a significant association between platelet count and treatment approach. However, there was no significant association between MPV and the treatment approach. Conclusion: Platelet parameters such as platelet count and mean platelet volume were not significantly correlated with NIHSS score, thus not a suitable additional parameter for assessment for acute ischemic stroke. However, unlike MPV, platelet count and MRI findings are useful parameters in guiding the treatment approach. A large scale involving multicenter study is recommended for further verification
