Abstract

in physical, psychological and social functioning occur with ageing. The dysfunction of multiple systems is the characteristic of frail people. In Malaysia, the data of multidimensional frailty are still limited and less studied. A cross-sectional study aimed to determine the prevalence of physical, psychological and social frailty and their associations with socio-demographic characteristics, anthropometric measures, multi-morbidities and lifestyle factors was conducted among 102 community-dwelling elderly (47.1% male; 52.9% female; mean age 66.6 ± 5.2 years; age ranged 60 – 82 years old) in Flat Taman Melati, Kuala Lumpur. Based on the cluster sampling, 3 out of 9 residential areas were randomly selected. The physical frailty was assessed using Fried’s criteria, whereas the psychological and social frailty were assessed using Tilburg Frailty Indicator (TFI). An interview-based questionnaire was utilized to collect data on socio-demographic characteristics, multi-morbidities and lifestyle factors such as smoking status and diet quality. Anthropometric measures included body mass index (BMI), body fat percentage, waist circumference (WC), mid-upper arm circumference (MUAC) and calf-circumference (CC). The prevalence of physical, psychological and social frailty among the respondents were 26.5%, 22.5% and 10.8%, respectively. In general, the respondents aged between 60 to 69 years (76.5%), unemployed/retired (46.0%) and had B40 income (88.2%). About 13.7% of them had poor diet quality and 49.0% had multi-morbidities. Majority of the respondents were overweight (50.0%), higher body fat percentage (95.1%) and abdominal obesity (58.8%). Female elderly with multi-morbidities and higher body fat percentage were associated with higher risk of being physically frail (p<0.05), whereas elderly with advanced age and lived alone were associated with higher risk of being socially frail (p<0.05). Moreover, those who have abdominal obesity were related with physical, psychological and social frailty (p<0.05). Other variables like MUAC and CC were linked with psychological and social frailty (p<0.05). In short, this study suggests that the physical, psychological and social frailty is prevalent in the community setting. Therefore, early preventive strategies focusing on multidimensional approach is warranted to minimise the frailty and its adverse outcome to this vulnerable age group.
