Abstract

Summary: Introduction: A cross sectional study was done among 143 oil palm plantation harvesters with the objective of determining the prevalence of musculoskeletal disorders (MSD) and the association with risk factors and productivity. Methodology: Nordic questionnaire was used in determining musculoskeletal disorders (MSD) symptoms and Rapid Entire Body Assessment (REBA) was used to determine the posture performed by the harvesters. Productivity is defined by 5 indicators (presenteeism, absenteeism, efficiency, daily harvesting income and daily quantity) in this study. Secondary data was obtained for daily harvesting income and daily quantity; data for other indicators were obtained through questionnaire. Median or mean score on productivity indicators was used as the cut -off point to dichotomize workers into group with productivity loss and group without productivity loss. Harvesters that had absenteeism (past month) or presenteeism (past week) were categorized to the group with productivity loss. Mean of group with productivity loss and without was compared to determine the mean reduction of productivity. Result: There is a high prevalence of MSD (93%) among the oil palm plantation harvesters. The REBA scores is high for most of the harvesters whereby 62.4% of them are in the high risk category and another 43% is in the very high risk category. There is a significant difference for efficiency (p=0.019) and the days of presenteeism (sick at work), p=O.OO between harvesters with MSD and without MSD. No significant association was found between risk factors and MSD but significant association was found between daily quantity (p=0.024) and presenteeism (p=0.006). Logistic regression adjusted for BMI, age and smoking also showed that harvesters with MSD (past 7 days) is 2.87 times more likely to have presenteeism and 2.09 more likely to be not productive in daily quantity compared to harvesters without MSD (past 7 days). Conclusion: There is no association between risk factors and MSD although there is significant association between presenteeism and quantity (work productivity indicators) with MSD. MSD is the predictor for reduced productivity in this study. Keywords: oil palm plantation, harvesters, musculoskeletal disorders, risk factors, productivity
