Abstract

A second year (Package 11) project dissertation. Project paper (Doctor of Medicine (MD)) - Universiti Putra Malaysia, 2013. Scientific Bac~round: Coronary artery disease (CAD) is defined as the narrowing of the coronary arteries. CAD deaths in Malaysia have reached 22,701 or 22.18% of total deaths. Exercise stress test (EST) using the treadmill exercise machine is an established focal point in the diagnosis of CAD. Currently, there is still no research done in Malaysia on the effectiveness of EST in diagnosing CAD. Obiectives: The aim of this study is to see the significance of EST in diagnosing CAD and Invasive Coronary Angiogram (ICA) that is the gold standard for diagnosing CAD was used as reference standard. Methods: A retrospective case series study was conducted using non-probability sampling method in electronic data bases (e-HIS Live). Data that are concerned in our research are filled in the proforma and then tabulated for analysis. The significance of EST in diagnosing CAD was described by sensitivity, specificity, likelihood ratio (LR), diagnostic odd ratio, positive predictive value (PPV) and negative predictive value (NPV) using standard formulas. The association between a pair of categorical data was examined by Chi-square, Fisher Exact, and odd ratio analysis. Frequency and percentage analysis were used to analyse a single variable. Results: This study found that there was no association between EST and ICA with p-value of 0.179. EST had a sensitivity of 87%, specificity of 22%, positive LR of 1.11, negative LR of 0.59, diagnostic odd ratio of 1.8, PPV of 0.0477 and NPV of 0.974 I. Conclusion: There are no significant association between EST and ICA. EST is a sensitive test in diagnosing CAD but it is not very specific. However, the high PPV for EST in this sample population is noteworthy and reassuring. Thus, EST is a cost-effective strategy for screening the common complaint of chest pain in high risk patients in primary care practices.
