Abstract
Introduction: Given that second-hand smoke (SHS) exposure poses health risks, smoking occurs frequently in Malaysian households, and it is almost exclusively a male behaviour. Research in urban areas shown that smoker are aware of the negative effects of SHS, nevertheless they may not necessarily implement smoke-free homes (SFH). There is a need to identify the level of understanding on SHS and SFH concepts to enable reduction of exposure to family members. Objectives: The objective of this study was to explore knowledge and understanding levels on SHS and implementation of SFH among male smokers in three FELDA settlement areas in the districts of Kuala Kubu Bharu and Muallim. Methodology: This was an explorative qualitative study carried out through focus group discussions (FGDs) from June to November 2023. Participants were current male adult smokers living in the settlement areas and snowball sampling method was used for recruitment. A topic guide for the FGD questions focusing on knowledge on SHS, myths and truths and intervention methods for smoke-free homes were developed with help from tobacco control experts. Data was assembled from FGDs and consisted of 10 respondents where FGDs were carried out in three sessions. Audio recordings were transcribed and translated in text and was exported into NVivo 14 for coding. Thematic analysis was performed using inductive methods in framework matrices. Approval for ethics were provided by the institutional review board of Universiti Putra Malaysia while the authority from FELDA was provided by the federal territory offices and appropriate FELDA offices at the research areas. Results: From the synthesis, participants had great knowledge and understanding on the tobacco harms and how they could be exposed to it as well as acceptance on SFH concepts however they had low knowledge SHS terms. Most participants were generally supportive of home and community-level SFH initiatives and highlighted awareness before its implementation. Community intervention includes declaration of SFH and introduction of modelling program by each settlement areas. Conclusion: There is good level of knowledge and understanding on SHS among smoking men in the FELDA areas. The support of smokers for SFH interventions indicated potential for enhancement. The government can incorporate data from this study to support and implement SFH policy, contributing to the accomplishments of the Malaysia Home Without Smoking.
