Abstract
Patients with End Stage Renal Disease (ESRD) who undergo hemodialysis (HD) treatment often had difficulties in managing their diet and fluids. This study determined the associations of socio-demographic, sodium intakes, perceived social support and perceptions on fluid restriction with fluid adherence among HD patients. A cross-sectional study was conducted among 92 HD patients in selected dialysis centres in Selangor, Malaysia. The patients completed interview- based questionnaires on socio-demographic, social support (Multidimentional Perceived Social Support Questionnaire) and perceptions on fluid restriction. A 2-day dietary data were obtained m and information on interdialytic weight gain (IDWG) were retrieved from medical records as secondary data. Half of the patients were aged between 41- 60 years old with 90% were married while 61% claimed that they have met with dietitian before and received nutrition advice. Over half (59.8%) had met the fluid allowance that is recommended for HD patients while only 29% had good perceptions towards fluid restriction. The proportion of patients who met recommended sodium intake was 70.7% and 81.5% had good IDWG. In general, the patients obtained moderate social support with perceived social support from friends was significantly lower than the other two subscales. There were significant correlations between IDWG with perceived social support (r= -0.023, p= 0.033), sodium intake (r= 0.302, p= 0.003) and perceptions on fluid restriction (r= -0.270, p= 0.009) among HD patients. There were also significant associations between ethnicity, educational level, marital status and fluid adherence among HD patients (p<0.001). The present study provides better insight on fluid adherence among HD patients for future measures and intervention. It is imperative to emphasize the restriction of sodium and fluid intake for the optimal control of fluid adherence among HD patients. Adequate social support from friends is crucial for the optimal control of fluid adherence. The understanding of the perceptions of fluid restriction suggesting that controlling fluid intake is very important. Further research regarding factors associated with fluid adherence among HD patients is recommended.
