Abstract

Poor appetite in hemodialysis patients can lead to multiple negative outcomes, including cachexia, increased mortality, impaired BMI, height, and weight, longer duration and rate of hospital, malnutrition and poor quality of life. There was a high prevalence of loss of appetite among hemodialysis patients globally, which was not congruent in Malaysia, probably due to the limited study in the local context. As such this cross-sectional study was performed with the aim of determining the magnitude of loss of appetite and its associations with selected factors. Socio-demographic characteristics, clinical background, appetite, depression status, and physical activity level among hemodialysis patients were assessed using self-developed questionnaire, BDI-II questionnaire and IPAQ questionnaire, respectively. Appetite status of participants were ascertained using ADAT questionnaire. Meanwhile, anthropometric measurements were self-measured with the aid of video demonstrations. Due to the Covid pandemic, this study was performed in a mixed mode (online survey and physical copies of questionnaires survey). Data were analysed using IBM SPSS Statistical version 25 with a significance level set at p<0.05. A total of 51 participants (27 male, 24 female) with a mean age of 49±15 years old participated in the study. Majority of the participants (69.2%) were below 60 years old, and a large portion of the participants were Malay (40.4%), married (63.5%) and unemployed (53.8%). Hypertension was most prevalent (66.7%) among the participants. Similarly, polypharmacy was prevalent with mean number of medications taken by participants was approximately six times a day. Most of the participants were sedentary (43.1%), while 25.1% were moderately active and 31.4% were active. Dual burden of malnutrition existed with approximately 45% of the participants had Body Mass Index (BMI) less than 23 kg/m2 (indicated protein energy wasting, PEW) while another approximately 12% with BMI of more than 30 kg/m2 (indicated obesity). On the other hand, muscle loss was prevalent among 47.1% of the participants while two-third of them had central obesity. Approximately 80% of the participants experienced good to very good appetite, while very few (7.8%) had poor appetite. Majority of the participants were not depressed (84.3%), but many of them presented with some degree of psychosocial burdens, which deserved attention. Using Chi-Square test of independence, there were significant associations between appetite with polypharmacy (χ2=14.355, p=0.002), and Type 2 diabetes mellitus (χ2=12.653, p=0.004). There were no significant associations between sociodemographic factors and comorbidities such as hypertension, anemia, hyperthyroidism, hyperlipidemia, dialysis vintage, BMI, calf-circumference, physical activity waist circumference and depression with appetite in hemodialysis patients. Despite the limitations enforced by the Covid-19 pandemic, the study has found significance associations between several factors with appetite among hemodialysis patients.
