Abstract
Malnutrition is a global crisis which defined as deficiencies or excesses in nutrient intake, imbalance of essential nutrients or impaired nutrient utilization; known to be the most common conditions that bring deleterious effect onto the health of elderly population due to presence of co-morbidities, cognitive impairment or decreased independence. Additionally, transitional care is described as action plan or discharge plan designed to ensure coordination and continuity of care especially between the health care and across care settings in which is frequently experienced by the elderly patients compared to patients of other age group. To date, research on transitional care of elderly in Malaysia is scarce and does not focus solely on addressing malnutrition issue. Therefore, the objective of this research was to study the transitional care in addressing malnutrition in elderly patients among healthcare professionals in Selangor. A cross-sectional study was conducted among 55 healthcare professionals from government health clinics and hospital located in Selangor selected through random sampling method starting from March till June 2021. Self-administered questionnaire was disseminated using online platforms for data collection. The questionnaire consisted of sociodemographic characteristics; practices, services and care components of transitional care of elderly; opinions and attitudes on transitional care of elderly; identification of the best practices and implementation strategies for transition care components in addressing elderly patients with malnutrition. A simple descriptive statistic was used to analyse the expand of transitional care in addressing malnutrition in elderly among the healthcare professionals in Selangor. Based on the analysis, majority of the respondents were able to determine the current practices, services and care components of transitional care with the mean score of 29.55 ± 3.701. Besides that, majority of them (81.5%) had shown positive opinions and attitudes towards transitional care of elderly with a mean score of 24.16 ± 3.14. The integration of malnutrition care (77.2%) and integration of community nutrition resources (88.6%) as part of transitional care components has shown positive advocation in identifying the best practices and implementation strategies. Regardless of the finding in this study, the establishment of guidelines and practices of transitional care in addressing malnutrition in elderly is still lacking. The future studies are suggested to focus on evaluating on the establishment of national guidelines and protocols for elderly transitional care in Malaysia’s healthcare sector, or the existing ones can be further evaluated for the effectiveness and importance in addressing the issues of malnutrition
