Abstract
Introduction: Herbal and traditional medicines are increasingly popular worldwide and continue to play an important role in health care in the modern world particularly, in developing countries. Aflatoxin B1 (AFB1) is a secondary fungal metabolite produced by primary fungus, Aspergillus flavus. It is widely found in herbal products and considered a public health concern worldwide due to its genotoxicity and carcinogenicity. Objective: The objective of this study is to determine the risk assessment of AFB1 in herbal and traditional medicine using the Margin of Exposure (MOE) approach. Methodology: Secondary data were collected from animal toxicological data and AFB1 exposure from the herbal products. Animal dataset with dichotomous hepatic cancer endpoint was analyzed using Benchmark Dose Software (BMDS) version 3.2 to derive the Benchmark Dose Lower Level of 10 (BMDL10). About 244 samples of AFB1 exposure data from herbal and traditional medicine were collected and the estimated daily intake (EDI) was calculated based on daily assumption of 4.87g of AFB1 and 70kg of body weight. Risk characterization was performed by calculation of MOE with the ratio of BMDL10 and EDI. Results and Discussion: Available literature data revealed that 117 out of 244 (48%) samples were contaminated with AFB1 above EU regulatory limit (>5 μg/kg). From this data, 226 out of 244 (92%) from the samples having MOE values less than 10, 000. Conclusion: Most of the herbal and traditional medicines contaminated with AFB1 were showed to have MOE less than 10, 000 indicating an urgency for the worldwide priority of risk management action. The production of herbal and traditional medicine should be monitored regularly to reduce the risk of AFB1 exposure.
