Abstract

Introduction: Necrotizing enterocolitis (NEC) is a gastrointestinal emergency which is associated with high mortality and morbidity in premature infants. Most of the times, patients will present with non-specific symptoms as well as abdominal signs including feeding intolerance, bloody stools and abdominal distension. Infant and maternal characteristics are also among the determinants for outcome of NEC. The aim of this study is to determine the incidence and clinical characteristics of NEC among preterm infants in Hospital Serdang from 2010 to 2019. Methods: This is a retrospective study using a data collection form or proforma to obtain secondary data. Medical records of both mothers and neonates for each case of NEC were retrieved via the hospital’s medical record database. The proforma records infant characteristics, neonatal clinical admission, maternal characteristics, diagnostic characteristics, neonatal outcome as well as sociodemographic factors. Universal sampling was used to select 49 preterm infants diagnosed with NEC. The data from medical records were recorded in the data sheets and analysed using SPSS version 26.0. Results: The overall incidence rate was 3.4% with the highest in the year 2010 (8.4%) and lowest in the year 2012 (1.31%). As for the infant characters of preterm infants with NEC, the most common characteristics were female (61.2%), Malay (73.5%), less than 32 weeks of gestation (49%), 32 weeks of gestation (28.6%) and birth weight less than 1000gm (40.8%). Next, the most common maternal characteristics of NEC were singleton (81.6%), born via LSCS (44.8%), dexamethasone administration (57.1%) and preeclampsia (28.6%). As for the clinical presentation, the most common presentation of NEC was abdominal distension (91.8%), dilated loop of bowel (81.6%), low haemoglobin level (57.1%), apnoea (24.5%), lethargy (22.4%) and temperature instability (18.4%). The overall mortality rate for NEC among preterm infants in Hospital Serdang is 30.61%. Significant characteristics associated vii with mortality were infant birth weight, gestation age, inotropic support, cord prolapse, mode of delivery, stagnant loop of bowel, portal venous gas, pneumoperitoneum and Bell’s criteria Conclusion: The incidence rate, mortality rate and common clinical characteristics in our study were comparable to the previous published studies
