Abstract

Background: Dizziness is a common complaint during a healthcare visit in the emergency department as well as outpatient clinics. It is defined as various sensations of body orientation and position that are frequently difficult to be described by patients, whereas imbalance can be described as the feeling of instability while being seated, standing, or walking without a specific directional preference. Dizziness can be further classified into vertigo, presyncope, disequilibrium and lightheadedness. It has been a burden for some people as it may affect their daily life and often leads to fall, especially in elderly above 60 years old. It also increases the risk of fracture, restricts mobility, depression, anxiety and even death in elderly. Based on previous studies, the prevalence of dizziness and imbalance in elderly is high in people aged more than 60 years old, in women, Chinese as well as those with low education levels. In addition, comorbidity factors such as diabetes, stroke and hypertension are seen to be more prominent to get dizziness and imbalance. 3 Significance of study: Previous cross-sectional studies have been carried out to measure the prevalence of dizziness and imbalance among the elderly which found that the prevalence has the tendency to increase with age. Thus, setting forth into this study in order to determine the factors associated with dizziness and imbalance amongst the healthy-ageing elderly population. Methodology: This is a cross- sectional study where online questionnaires will be distributed to the members of University of Third Age (U3A), Malaysia. We included all elderlies above 60 years old that have the ability to use smartphones or email, and are able to understand English language or Bahasa Melayu. AMER Dizziness Diagnostic Scale (ADDS) and Activities- specific Balance Confidence (ABC) Scale are used in our study where it helps with the differential diagnosis and assessing the confidence level in elderly respectively. These questionnaires are converted into Google Form and will be distributed to our targeted population via online platforms. The statistical software, Statistical Package for Social Sciences (SPSS) version 25.0 (SPSS Inc.,Chicago, IL, USA) will be used to analyze all the data collected and extracted in this study. Data will be expressed as mean ± standard deviation. Mann-Whitney-U and Kruskal-Wallis are used as a test to perform comparison between independent variables (age, gender, and race) and dependent variable (prevalence of dizziness and imbalance in healthy ageing elderly and level of functioning of elderly). Results: Out of 200 respondents, the prevalence of dizziness and imbalance among elderly was 63.5%. There is a significant association between hypertension and neurological problem with the ADDS Questionnaire while ABC Scale is also significantly associated with sociodemographic factor ‘race’. We also found that dizziness and imbalance have an impact on level of functioning among elderly (p < 0.05).
