Abstract

Dengue alone is a complex disease involve immunologic reactions between host genetic factors, comorbidities, and viral strains that can lead to mortality mainly in elderly. Bacterial infection is the most common type of co-infection in dengue cases that can cause evere outcome. Besides that, disease severity is also influenced by comorbidity. Comorbidity is the presence of one or more than one disorder in the same person that increases the risk of medical conditions with further morbidity and mortality. Both bacterial co-infection and comorbidity could worsen the severity of dengue progression. This study aims to identify the common comorbidity in dengue with bacteria co-infection and to investigate the relationship between comorbidity with severity of the disease in dengue with bacteria co-infection during the dengue outbreak in 2016. There is correlation between the comorbidity with severity of the disease in dengue with bacteria co-infection. A total of 72 dengue patients with bacteria co-infection were extracted from 1730 confirmed dengue cases from Hospital Serdang data system. Then, the comorbidity of these patients was extracted. Oversampling methods was applied to increase the datasets 5 times bigger. Charlson’s Comorbidity Index (CCI) and SOFA score were calculated using online calculator “mdcalc.com”. Next, Chi-square was used to determine association between the two categorical variables. A p-value of less than 0.05 were considered as statistically significant. Among the 72 patients, the common comorbidity is obesity (16.7%) but not associated with severity of dengue. While diabetes mellitus (12.5%), followed by hypertension (9.7%), cardiovascular (2.8%), stroke (2.8%), chronic kidney diseases (2.8%), and dyslipidemia (1.4%) had shown significant association with the severity of dengue. Besides that, high SOFA score (6-8) and CCI score (above 2) also shown significant association with the severity of dengue. Obesity is common due to their lifestyle behavior. Diabetes and hypertension patients are prone to be infected by dengue with bacterial infection due to the metabolic syndrome presented that leads to immunocompromised. Based on SOFA score, the severity of dengue is associated with various organ dysfunction due to bacteria invasion and immune system disruption. The association between comorbidity with severity of the disease in dengue with bacteria co-infection can help the clinician to predict the disease progression, thus provide prompt and proper treatments for dengue patient.
