Abstract

Introduction: In Malaysia, non-smoking females had a higher risk of being exposed to second-hand smoke (SHS) compared to males, especially in a non-smoke-free home. Studies have highlighted the main negative impacts of SHS exposure such as lung cancer and asthma. Objective: This research aims to explore the experiences, perceptions and knowledge on SHS, smoke-free home (SFH) and smoke-free interventions among non-smoking females and female leaders in FELDA settlement areas in districts of Kuala Kubu Bharu and Muallim. Methodology: An exploratory qualitative study was conducted involving 16 key informants via snowball method at four different FELDAs in the districts of Kuala Kubu Bharu and Muallim. A semistructured topic guide was developed and has four main parts to explore the understanding of SHS and facilitate a smoke-free home. The part discussed was understanding terms, facts and myths statement on SHS where they need to identify whether they believe the statement to be right or wrong, various types of intervention that were closely related to the implementation of smoke-free homes, and smoke cessation services available. Four different sessions of Focus Group Discussion (FGD) were conducted in the FELDAs and the audio recordings for the FGDs were professionally transcribed and translated and were then analysed using NVIVO 14. Framework matrices were developed and were used to create a summary for the data synthesis. Result: In terms of knowledge, three out of four groups have a limited understanding of the term SHS and smoke-free homes. As for the myths and truths on SHS, all groups strongly agree that even one stick of smoke can harm the smoker and people in the surroundings. All groups mentioned that advising the smoker to smoke outside was not effective. In terms of perception of a smoke-free home, air quality monitoring and declaring a smoke-free home intervention method were fully supported by the participants. Other smoke-free home intervention strategies were generally supported by the participants but further improvement can be done to increase its effectiveness. Lastly, all participants believed the smoking cessation services that were provided by the health care were not effective for quitting smoking. Conclusion: Majority of participants had poor knowledge of the meaning of the term SHS and passive smoking but good knowledge on harm of SHS exposure. The participants perceived smoking cessation program to be less effective, thus fully supported the implementation of SFH interventions in their community.
