Abstract
Introduction: Malnutrition is one of the most common comorbidities among cancer patients, which can have adverse impacts to patient clinical outcomes. To date, researches on gynecological cancer (GC) in Malaysia is scarce and does not focus solely on nutritional status and functional status of patients. The objective of this study was to determine prevalence of malnutrition among GC patients and investigate the association between nutritional status and functional status with malnutrition among GC patients. Methods: A cross-sectional study was conducted among 80 patients from Institut Kanser Negara, Putrajaya from January to February 2020. Interviewer-administered questionnaire was used to collect the data that included socio-demographic background, medical history, anthropometric measurements, biochemical data, dietary data, functional status, while malnutrition status was assessed using Scored Patient-Generated Subjective Global Assessment (PG-SGA). Results: Prevalence of malnutrition using scored PG-SGA was 38.8%. Most subjects were Malay with age range of 19 to 79 years and mean age of subjects of 55.50 ± 14.16 years old. Majority of them had normal Body Mass Index (BMI) (41.3%) and did not have any significant weight loss in one month (71.3%). Most subjects were estimated having adequate energy intake (76.3%) and inadequate protein intake (82.5%) based on Medical Nutrition Therapy guideline for cancer patients. There were significant associations between BMI, weight change in one month, albumin, and energy intake with malnutrition when tested using chi-squared test of associations (p<0.05). Meanwhile, Pearson’s correlation test showed that lower BMI value (r=-0.333, p=0.003), lower white blood cell (r=-0.256, p=0.022), and lower hand grip strength (r=-0.266, p=0.017) were correlated with higher risk of malnutrition (high mean score of PG-SGA). Conclusion: Prevalence of malnutrition in GC patients is lower compared to other existing local and international studies. However, current study finding highlighted the importance of carrying out malnutrition detection earlier to provide prompt nutrition intervention to GC patients.
