Abstract

Increasing trends of elevated blood pressure among older people has resulted in a significant number of morbidity and mortality. This cross-sectional study aimed to determine the prevalence of hypertension and its associated factors with blood pressure among hospitalized elderly in Hospital Serdang, Selangor. A total of 124 hospitalized elderly in Hospital Serdang Selangor was selected through purposive sampling. A complete set of the self-developed questionnaire consists of food history, Mini Nutritional Assessment Short-Form (MNA-SF) and Geriatric Depression Scale (GDS). There were 59.7% were male and 40.3% were female with mean age of 66.81 ± 5.35 years. Majority were found to have hypertension (72.6%) with mean blood pressure 133.15 ± 16.49 mmHG for systolic BP and 74.46 ± 9.591 mmHg for diastolic BP, while approximately 68.5% reported with polypharmacy. This study found 38.7% having a normal body mass index (BMI). Majority presented with normal lipid profile and potassium level but about half of them found with abnormal fasting blood glucose (58.1%) and creatinine level (50.8%). Majority of the subjects also reported with insufficient dietary intake except for trans-fat, sodium and caffeine. Based on total score for MNA-SF, 66.1% having normal nutritional status. Among the subjects, 62.1% engaged with physical activity, 23.4% were smoking and 4.8% were taking alcohol. Most of the subjects (86.3%) found with normal stress level for GDS score. In this study, prevalence of hypertension in Hospital Serdang, Selangor was 72.6% and factors such as length of stay, number of comorbidities, number of medications and having comorbidities of hypertension, dyslipidemia, and diabetes mellitus, polypharmacy, height, BMI, total cholesterol, fasting blood glucose, fiber, polyunsaturated fat, dietary cholesterol, caffeine and duration of physical activity were significantly associated with the blood pressure at p<0.05. In conclusion, a good understanding of managing factors associated with elevated blood pressure should be enhanced to reduce the morbidity and mortality among the elderly.
