Abstract
Protein energy wasting (PEW) is a common complication among chronic kidney disease. Inadequate protein (DPI) and energy intake (TEI) are common cause to PEW. However, the factors associated with inadequate DPI and TEI often goes unrecognized among peritoneal dialysis (PD) patients. Thus, this cross-sectional study aimed to determine dietary protein and energy intake and its associated factors among PD patients in government hospitals. Sociodemographic questionnaire and dental status were obtained from semi-quantitative questionnaire, medical history and biochemical data retrieved from patient’s medical history while anthropometry measurement and handgrip strength performed by the researcher. Dietary intake was assessed through three days diet recall. Appetite assessment were assessed by using Appetite and Dietary Assessment Tools (ADAT). A total 104 PD patients (45% male and 55% female) with the median age 57.0(20) years old and achieved secondary education level (50%) were participated. Majority of the patients shown a high level of creatinine (859.51 ± 272.98 μmol/L). The appetite of the patients was good, fair and very good (30%, 29% and 19%) while 22% had poor appetite. 65% of the patients were non-denture wearer. For handgrip strength, the median was 16.95(8.8) kg. Majority patients had inadequate protein intake (73%) however shown to have adequate energy intake (58%) than recommended. Age, creatinine and handgrip strength shown a significant association with TEI (p<0.01) while appetite assessment shown association with DPI (p<0.05). Early intervention plan can be considered to monitor patients with inadequate DPI and TEI to prevent higher risk of mortality and morbidity and subsequently will lead to PEW.
