Abstract

Sarcopenia is known as a condition of progressive and widespread loss of skeletal muscle mass, strength, and function among older adults. Besides aging process, insulin resistance, which is related to diabetes mellitus, is one of the risk factors that is associated with the loss of muscle mass and function. Previous studies had shown high prevalence of sarcopenia among community-dwelling elderly population not only in Malaysia but also in other country. However, study on sarcopenia status among elderly patients with diabetes mellitus is still limited in Malaysia. Thus, this study aimed to determine the sarcopenia status and its associated factors among elderly patients with diabetes mellitus in Medical Outpatient Clinic of Hospital Serdang, Selangor. This was a cross-sectional study which used a non-probability sampling design. A total of 68 elderly patients with the mean age of 69.37 ± 5.84 years old were recruited from Medical Outpatient Clinic in Hospital Serdang. The criteria of Asian Working Group of Sarcopenia (AWGS) in 2014 was used to determine the sarcopenia status. Self-administered questionnaire and face-to-face interview were used to assess sociodemographic characteristics, nutritional risk status (Mini Nutritional Status-Short Form), health-related status and frailty status (Fried Criteria). For anthropometry characteristics, weight, height, body mass index (BMI), body fat percentage (%), skeletal muscle mass (SMM), hand-grip strength, and gait speed measurement were obtained. By using the AWGS criteria, 42.6% of the elderly patients had sarcopenia. Ethnicity ( ꭓ2=5.275, p=0.022) and BMI status ( ꭓ2=5.537, p=0.019) were significantly associated with sarcopenia. However, there was no significant association were found between other sociodemographic characteristics, anthropometry characteristics, nutritional-risk status, health-related status and frailty status with sarcopenia (p>0.05). In conclusion, a more in-depth study need to be done to find the associated factors with sarcopenia among elderly patients with diabetes mellitus. For further studies, larger sample size should be use to refer to sarcopenia status in the whole populations of elderly patients with diabetes mellitus in Malaysia.
