Abstract

Fluid adherence had been reported as the most challenging regimen among hemodialysis (HD) patient to accomplish, especially in a tropical setting like Malaysia. However, studies on fluid adherence and its associated factors among Malaysian HD patients is highly scarce. Thus, this cross-sectional study aimed to investigate the factors associated with fluid adherence among HD patients in Selangor. A total of 81 subjects (49 males, 32 females) with the mean age of 59 ± 14 years old were recruited from two hemodialysis centers in Selangor, Malaysia. Face-to-face interviews were conducted to ascertain information on sociodemographic and other variables including presence of comorbidities, dialysis vintage, total duration of dialysis treatment per week, height and dry weight of the patients. Hospital Anxiety and Depression Scale (HADS), Multidimensional Scale of Perceived Social Support (MSPSS) and Indonesian Fluid Intake Appraisal Inventory (I-FIAI) were used to assess anxiety and depression level, perceived social support and self-efficacy on fluid adherence, respectively. All data were analysed using IBM SPSS Statistics 26 with the significance level set at p<0.05 Majority of subjects were Chinese, had secondary education, married and retired. The prevalence of fluid non-adherence was 32.1% with a mean interdialytic weight gain of 3.45%. A higher level of fluid adherence was significantly associated with an older age, unemployed and female patients (p<0.05). However, this study failed to demonstrate significant association between fluid adherence and clinical factors, anthropometry parameters, psychology factors and self-efficacy factor. Given the consistently documented low fluid adherence among HD patients in Malaysia, the relevant authorities are recommended to dedicate greater attention and emphasis to developing effective interventions for these patients in order to avoid unfavourable health effects.
